Important: Complete the McKinney-Vento Housing Questionnaire first during registration. If your student is in temporary or unstable housing, please contact your school office so that staff can assist with enrollment and available services.

Welcome! This page provides access to essential registration forms used by Hamilton Community Schools. Forms with “PS” preceding the name are available electronically through the parent’s PowerSchool account.

Important – Housing Questionnaire
All families must complete the Housing Questionnaire as part of student registration. This form helps the district identify students who may qualify for additional services under the McKinney-Vento Homeless Assistance Act.

If your student is currently living in temporary or unstable housing (for example, staying with relatives or friends, in a motel, shelter, campground, car, or another temporary arrangement), please contact your school office before completing the remaining registration forms. School staff will assist you with the enrollment process and ensure your student receives any services they may qualify for.

Form Categories:

Electronic Forms (contains all required, situational, and medical forms)
Available through the PowerSchool Parent Portal. Complete using a computer or mobile device.

Required Forms
Must be submitted for each student before the start of the school year or upon enrollment.

Situational Forms
Only needed under specific circumstances (e.g., custody, transportation changes).

Medical Forms
Used to communicate health conditions or medications with the school nurse.

Athletic Forms
Required for any student participating in school-sponsored sports.

How to Use These Forms:

While we recommend completing forms electronically through the HCS Returning Student Registration link, all forms are available as PDFs. You may fill out a form electronically, then print and sign it, or print a blank form and complete it by hand.

Submission Guidelines:

Submit all required forms before the first day of school or as part of new-student enrollment.
Complete situational, medical, and athletic forms as needed throughout the year.
