Welcome! This page provides access to essential registration forms used by Hamilton Community Schools. Forms with a 'PS' preceding the name are available as electronic forms through the parent's PowerSchool account.

Form Categories:

Electronic Forms
Available through the PowerSchool Parent Portal. Complete using a computer or mobile device.

Required Forms
Must be submitted for each student before the start of the school year or upon enrollment.

Situational Forms
Only needed under specific circumstances (e.g., custody, transportation changes).

Medical Forms
Used to communicate health conditions or medications with the school nurse.

Athletic Forms
Required for any student participating in school-sponsored sports.

How to Use These Forms

While we recommend completing forms electronically through the HCS Returning Student Registration link, all forms are available as PDFs. You may:

  • Fill out a form electronically, then print and sign it

  • Print a blank form and complete it by hand

Submission Guidelines

  • Submit all required forms before the first day of school or as part of a new student enrollment.

  • Complete situational, medical and athletic forms as needed throughout the year.
